RESUMEN

Background: The transition to parenthood is one of the most challenging experiences in a couple's life, which can be stressful and difficult. A positive transition period affects the quality of parents' behavior and the baby's health. This qualitative study aimed to explain the educational needs of adaptation to parental role among first-time parents in Iran. Materials and Methods: In this qualitative study, 25 participants from a variety of ethnic backgrounds were recruited in Ahvaz, Iran, using purposive sampling. In-depth interviews were used to collect the data which were analyzed by qualitative content analysis. Results: Three main categories emerged from the data analysis: "The need for knowledge improvement training," "The need for psychological adaptation training," and "The need for sociocultural adaptation training." Conclusions: To adapt to the parental role, first-time parents should be equipped with the knowledge to turn the challenges of this period into an opportunity for growth. Moreover, they need to be supported by their family members, the healthcare team, and the government.

PURPOSE: Our study aimed to translate the Stuttering Generalization Self-Measure (SGSM) into Persian and investigate its validity, reliability, and internal responsiveness in the Iranian population. METHOD: This study was conducted on 30 adults who stutter (AWS) and 30 adults who do not stutter (AWNS). The International Quality of Life Assessment protocol (IQOLA) was applied to translate SGSM into Persian. The face and content validity were determined. Also, the discriminant validity was evaluated by comparing the scores of two groups. In addition, the internal consistency test-retest, and inter-judge reliability were assessed with Cronbach's alpha and intra-class correlation coefficients (ICC). Moreover, the mean standardized response (MSR) and the standardized effect size were measured to determine the internal responsiveness using pre- and post-treatment data. RESULTS: All the items were comprehensible and clear. The content validity ratio (CVR) and content validity index (CVI) for all nine questions were obtained higher than.62 and.9, respectively. The internal consistency value was high (Cronbach's alpha =.98). For the test-retest reliability, ICC values were excellent, ranging from.93 to.99. The discriminant validity results revealed a significant difference between AWS and AWNS (p < .001). Pre- and post-treatment results indicated high internal responsiveness to changes for percentage of syllable stuttered (SS%) (MSR = 1.09). CONCLUSION: The Persian version of SGSM (P-SGSM) benefits from the high values for validity and reliability. Furthermore, it distinguishes the AWS and AWNS and reflects the treatment changes significantly.

The lichen Usnea articulata collected from an unpolluted area was exposed for 6 months at 26 sites for the sample chosenusing a stratified random design, and the content of potentially toxic elements (PTEs) including As, Cd, Cr, Cu, Fe, Hg, Mn, Mo, Ni, Pb, Sn, V, and Zn, was assessed using ICP-MS. The health risk for both adults and children was then calculated using the PTEs concentrations. The results showed that despite the hostile urban conditions, transplanted lichens depicted clear deposition patterns of airborne PTEs, mostly associated with industrial sites, where As and other elements showed remarkably high values. The cumulative hazard index was below the risk threshold, both for adults and children. For the entire population (particularly children) residing in areas surrounding industrial sites, As and Cr appeared to be potentially carcinogenic elements.

Background: The maternal sense of competence refers to the mother's sense of ability to care for the baby. Maternal competence can affect the mother's parenting capacities. Maternal preparation programs are aimed to increase mothers' knowledge and improve their maternal competence. This systematic review and meta-analysis investigated the effect of a maternal preparation program on maternal role competence. Materials and Methods: A systematic search was conducted using the following MeSH terms: "education", "program", and "parenting sense of competence" in databases including PubMed, Web of Science, Cochrane Central, and Scopus, from inception till July 2022. All Randomized Controlled Trials (RCTs) published in any language were extracted. Articles were screened based on predefined inclusion and exclusion criteria. The quality of the included articles was assessed by two qualified reviewers based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Results: Of the 170 published works that were retrieved in the initial stage, five articles including 647 first-time mothers were analyzed. A meta-analysis showed that parenting preparation program interventions increased maternal role competence in first-time mothers as opposed to those who received routine care Mean Difference (MD) = 3.31, 95% Confidence Interval (CI) [2.07, 4.55]. Conclusions: The results of this study encourage health policymakers to develop maternal preparation programs for first-time mothers and their children.

Background: Varicocele is an abnormal dilation and enlargement of the scrotal venous pampiniform plexus that impairs normal blood drainage and finally leads to infertility if not treated. Objective: This study aimed to figure out the impact of mitochondria status through the mitochondrial membrane potential (MMP) and adenosine triphosphate (ATP) assessment and its correlation with semen parameters to illuminate the impact of sperm mitochondria healthiness on normal sperm functionality. Materials and Methods: This analytical cross-sectional study was conducted with 100 men including 50 cases in the normozoospermic group (normal) and 50 in an infertile group with the non-varicocelectomy operation (varicocele) referring to Infertility Research and Treatment Center, ACECR Khuzestan, Iran. Routine semen analysis was performed according to World Health Organization guidelines, DNA fragmentation index, the MMP assay, ATP content, and apoptosis were carried out for all samples. Results: The results showed that the concentration, progressive motility, normal morphology, MMP, and ATP contents of sperm in varicocele were significantly lower than the normal group. In addition, the sperm DNA fragmentation index was significantly higher in the varicocele group in comparison with the normal group. Conclusion: Reduction in MMP and ATP contents, besides the loss of sperm parameters quality and increase in sperm DNA fragmentation, were seriously implicating sperm mitochondria dysfunctionality in varicocele men.

Introduction: There is a controversy regarding the relationship between blood eosinophil count and COPD exacerbation. We aimed to determine whether peripheral eosinophils upon COPD diagnosis could affect the frequency and severity of annual acute exacerbation of COPD (AECOPD). Methods: This prospective study was conducted on 973 newly diagnosed COPD patients who were under 1-year follow-up in a pulmonology center in Iran. The Cox proportional model, polynomial regression, and receiver operator characteristic curves were conducted to evaluate the impact of the eosinophil levels on AECOPD. A linear regression model was conducted to evaluate the continuous association of eosinophilic count with AECOPDs. Results: Patients with eosinophil >200 cells/microliter were higher pack-year smokers with more pulmonary hypertension prevalence compared to COPD patients with <200 cells/microliter. There was a positive correlation between the eosinophilic count and the frequency of AECOPDs. Eosinophil >900 cells/microliter and eosinophil >600 cells/microliter had a sensitivity of 71.1% and 64.3%, respectively, in predicting the occurrence of more than one AECOPD. Eosinophilic count cutoff of 800 cells/microliter had the highest Youden index with sensitivity and specificity of 80.2% and 76.6%, respectively, for incident AECOPD in newly diagnosed patients. Using a linear model, increasing 180 cells/microliter in serum eosinophils was associated with further exacerbation. Evaluating gender, BMI, smoking pack-year, FEV1/FVC, CAT score, GOLD score, pulmonary hypertension, annual influenza, pneumococcal vaccinations, leukocytosis, and blood eosinophils, only blood eosinophils (hazard ratio (HR) = 1.44; 95% confidence interval = 1.33-2.15; p value = 0.03) and GOLD score (HR = 1.19; 95% CI = 1.30-1.52; p value = 0.03) were found as independent risk factors of AECOPD >3 episodes/year. Requirement for ICU admission, invasive ventilation, and mortality rate due to AECOPDs was similar between eosinophilic and noneosinophilic groups. Conclusion: Eosinophilia upon COPD diagnosis is a factor of recurrent AECOPDs. To reduce the risk of AECOPDs and the burden of disease, clinicians may consider inhaler corticosteroids and domiciliary oxygen with a lower threshold for eosinophilic-COPD patients regardless of their clinical status.

BACKGROUND: Placenta accreta spectrum (PAS) is one of the life-threatening complications of pregnancy, the prevalence of which has increased in parallel with the caesarean section rate. OBJECTIVE: The purpose of this study was to investigate the experiences of mothers with PAS who have also experienced maternal near miss. METHODS: The participants of this study included 8 mothers who had experienced near miss due to placenta accreta during the past year, as well as two husbands and two health care professionals. Data collection was done using face-to-face, in-depth virtual and in-person interviews. In this qualitative study, the interpretive phenomenological analysis approach was used to analyse the data. RESULTS: The superordinate theme that emerged from the lived experiences of the studied mothers is 'Living in a vacuum', which was derived from 3 main themes. The theme of 'distorted identity' is related to the mothers' experience of losing the uterus as a symbol of femininity and nostalgia for the former self. The theme of 'exacerbated exhaustion' indicates the burnout and fatigue perceived by these mothers and has dimensions far beyond the exhaustion caused by performing parenting duties. The third theme, 'a threatened future', reflects these mothers' vague image of the future in terms of health, preservation of life, and the continuation of living together with the husband. CONCLUSIONS: It seems that mothers with PAS need to be covered by integrated and well-organised psycho-social support from the time they are diagnosed with the complication until long after delivery due to the high potentiality of maternal near miss.

Objective: Perineal trauma is a serious and frequent problem after childbirth which is experienced by millions of women worldwide. The technique used for perineal repair may have an impact on pain and wound healing. The aim of the present study was to compare the continuous non-locking technique with interrupted suturing for the repair of episiotomy or second-degree perineal tears. Methods: A single-blind randomized-controlled trial was conducted from October 2021 to August 2022 in Sina Hospital, Ahvaz, Iran. Three hundred women were selected and randomly assigned into control and intervention groups using block randomization technique. The main outcomes included pain and wound healing that were assessed using visual analog scale (VAS), as well as redness, edema, ecchymosis/bruising, discharge, and approximation scale (REEDA). The secondary outcomes were the use of analgesics, duration of perineal repair, material used for suturing, pain during urination and defecation, and resumption of sexual intercourse. The participants were followed up on the first and seventh days and in the 6th week postpartum. Mann-Whitney, Chi-square, and Generalized Estimating Equations (GEE) model were used for data analysis. Results: Wound healing was significantly better in the continuous non-locking suture technique compared to the interrupted technique (ß = -1.98; P > 0.0001). Women also experienced less pain in the continuous non-locking suture technique (ß = -2.46; P > 0.0001). There was a reduction in the use of analgesics, the duration of perineal repair, and the material used for suturing in the continuous non-locking suturing technique as opposed to the interrupted method (P < 0.0001).The odds of pain during urination and defecation significantly reduced in women who underwent the continuous non-locking method (P < 0.001). Also, women in the continuous non-locking group resumed their sexual intercourse earlier (P < 0.0001). Conclusion: The findings of this study revealed that use of continuous non-locking technique for suturing was associated with reduced perineal pain and improved wound healing. Furthermore, it was associated with a shorter duration of perineal repair, less suture material used, and less need for analgesics compared with the interrupted method. BACKGROUND: There is no study in the world on the relationship between consuming black and green tea as beverages containing polyphenols and the risk of MS. This study aimed to determine the association between the consumption of green and black tea, coffee, non-alcoholic beer, milk, fruit juices and carbonated beverages with the risk of MS. METHODS AND MATERIALS: This case-control study was performed on 150 patients with MS and 300 healthy individuals as a control group among patients who were referred to the ophthalmology ward of a referral hospital in Ahvaz with the groups matching for age. The data collection tool was a researcher-made questionnaire including demographic information and beverage consumption. Analysis was performed using univariate and multiple logistic regression models. RESULTS: The mean age of patients at the time of diagnosis was 38.55 ± 8.88 years. The results showed that drinking milk (OR = 5.46), natural juice (OR = 2.49), and carbonated beverages (OR = 16.17) were associated with an increased chance of developing MS. However, drinking non-alcoholic beer (OR = 0.48), black tea (OR = 0.20), green tea (OR = 0.29) and coffee (OR = 0.07) were associated with a reduced chance of developing MS. CONCLUSION: The results show that drinking black and green tea, non-alcoholic beer, and coffee are associated with a decrease in the chance of developing MS. The results of this study can be used to design interventional research and to change people's lifestyles to prevent MS.

BACKGROUND AND AIM: Pomegranate as a functional food has various properties and effects on health. The aim of the study was to evaluate the effect of pomegranate extract on serum levels of liver enzymes, hepatokines, interleukin-6 (IL-6), and total antioxidant capacity in non-alcoholic fatty liver disease (NAFLD). METHODS: In this double-blind randomized clinical trial, 44 patients with NAFLD were divided into two groups: pomegranate extract tablets and placebo. The intervention period was 12 weeks. At the beginning and end of the study, serum levels of alanine transaminase (ALT), aspartate transaminase (AST), gamma-glutamyl transferase (GGT), alkaline phosphatase (ALP), fetuin-A, fibroblast growth factor 21 (FGF-21), interleukin-6 (IL-6), and total antioxidant capacity were assessed in both groups. RESULTS: Pomegranate extract reduced the level of ALT (P < 0.001), AST (P < 0.001), GGT (P < 0.001), fetuin-A (P < 0.001), FGF-21(P < 0.001) and IL-6 (P = 0.04) compared to the placebo. Pomegranate extract also led to an increase in total antioxidant capacity (P˂0.001) but had no effect on ALP. CONCLUSION: It seems that the pomegranate extract improves several markers of NAFLD, and can be useful as a treatment supplement. Objectives. This study aimed to determine the prediction of risk of coronary artery disease (CAD) based on the Framingham risk score (FRS) in association with shift work among nurses.Methods. This cross-sectional and descriptive-analytic study was conducted with nurses working in three hospitals located in Abadan, southwest Iran. The sample size was 120 participants, divided into 60 shift workers and 60 day workers (non-shift workers). Demographic data and medical and occupational history were collected through a checklist and interviews. CAD risk assessment tools used to estimate the 10-year cardiovascular disease (CVD) risk included the FRS. Data were analysed by Mann-Whitney U test, χ2 test and one-way analysis of variance (ANOVA) using SPSS version 22.0.Results. There were statistically significant differences between blood pressure (BP), total cholesterol (TC), body mass index (BMI) and shift work (p ≤ 0.001). There was a statistically significant difference between prevalence of CAD risk and shift work based on the FRS (p = 0.04).Conclusion. The study findings showed that shift work is a risk factor for CAD disturbances. Therefore, particular follow-up of shift workers should be recommended for screening and preventing CAD.

Introduction: During the COVID-19 pandemic, many changes occurred in various cultural, social, and economic fields, leading to the creation of psychological effects, especially anxiety, in the community. Fear and anxiety about emerging diseases (COVID-19) and less participation in preventive behaviors reduce individual resistance and ultimately lower one's quality of life. Therefore, we aimed to investigate the relationship between health literacy and generalized anxiety disorder during the COVID-19 pandemic in Khuzestan province, Iran. Methods: This was a descriptive-analytical (cross-sectional) study conducted among participants aged 18-65 in Khuzestan province during the years 2020-2021 through online sampling. Due to the prevalence of COVID-19 and the impossibility of face-to-face communication, the questionnaire was designed on the Porseline Survey website and the questionnaire link was provided to participants through WhatsApp and Telegram. The data collection tool includes the Iranian Health Literacy Questionnaire, which includes 33 items, and the Generalized Anxiety Disorder-7 scale, which has 7 items based on a Likert scale. Data analysis was performed using STATA14 software and descriptive and analytical statistical tests at a significant level less than 0.05. Results: The mean (standard deviation) score for health literacy was 52.9 ± 9.3 and for generalized anxiety disorder was 5.2 ± 3.1. A significant negative correlation was found between the mean of anxiety disorder and health literacy (p < 0.05). The frequency of health literacy in individuals included 427 (37.8%) with inadequate health literacy, 628 (55.6%) with marginal health literacy, and 75 (6.6%) with sufficient health literacy. There was a statistically significant difference between the mean of generalized anxiety disorder among individuals with different levels of health literacy (p < 0.05). Conclusion: As the level of health literacy increases, the prevalence of generalized anxiety disorder caused by fear of COVID-19 decreases. Therefore, increasing awareness and health literacy about this virus, its transmission, and prevention methods is very effective in managing anxiety and stress caused by COVID-19. Paying attention to the issue of health literacy and generalized anxiety disorder, and promoting preventive behaviors can be effective tools for planners, health officials, and policymakers to promote health literacy for any type of disease.

Research objectives: Intertwined with modern life, air pollution is not a new phenomenon. Air pollution imposes a significant number of deaths and disease complications on society, and therefore it is very important to determine the extent of its effects on health in any society. This study sought to evaluate the concentration and short-term and long-term excess mortality attributed to PM2.5, NO2 and O3 observed in Shahrekord. Procedure: Hourly concentrations of PM2.5, O3, and NO2 measured at different stations of the Shahrekord Monitoring Network were obtained from the Shahrekord Department of Environment (DOE). Then, for different air quality monitoring stations, the average 24-hour PM2.5 concentration, the one-hour average NO2 concentration and the maximum 8-hour daily O3 concentration were calculated using Excel 2010. When the maximum 8-hour daily ozone level exceeds 35, it drops below 35 to calculate the SOMO35 index for modeling. Results: The death rates of IHD, COPD, lung cancer and ALRI and stroke related to PM2.5 were 176, 7, 0, 10, 105, respectively. The effect of ozone on respiratory mortality was zero. During the study period in Shahrekord, no respiratory mortality was determined due to ozone and acute lower respiratory tract infection (ALRI). this study is first ever study on health effects of air pollution in shahrekord city. Conclusion: A significant number of deaths due to air pollutants in Shahrekord have been reported. It can be concluded that by designing and implementing strategies and measures to control air pollution, both health effects and economic losses are prevented.

Background: Placenta accreta spectrum (PAS) disorder is an important life-threatening problem. The purpose of the current study was to determine the frequency, risk factors, and pregnancy outcomes of PAS in our population. Methods: This is a case-control study using the data from a main tertiary referral university hospital in Ahvaz, southwest of Iran. The sample included 187 cases diagnosed with placenta accreta spectrum from 2015 to 2019 and 552 controls without PAS. A multivariable logistic regression model was used to find independent risk factors with 95% confidence interval. Pregnancy outcomes were evaluated using chi-square, t-test, and Mann-Whitney U test and p<0.05 were considered statistically significant. Results: The frequency of PAS during the study period was 3.7/1000 deliveries (0.37%). It was found that multiparity (≥3 deliveries, OR=2.05: 95%CI:1.21-3.47) and multigravidity (≥3 deliveries, OR=2.98: 95%CI:1.55-5.72), prior cesarean delivery (OR=52.55: 95%CI:19.73-139.96), and placenta previa (OR=27.48: 95%CI: 9.62-78.5) are the independent risk factors of PAS. Complications and morbidity associated with PAS included hysterectomy (60.4% vs. 0.7%, p<0.001), cystostomy (24.1% vs. 0.2%, p<0.001), the need for blood transfusion (73.7% vs. 1.4%, p<0.001), intensive care unit admission of mother (42.8% vs. 0.2%, p<0.001), duration of hospitalization (7.52±6.34 vs. 1.97±1.83, p<0.001), preterm birth <37 weeks (61.4% vs. 16.8%, p<0.001), and perinatal mortality (7.4% vs. 1.8%, p<0.001) which manifested statistically significant values. Conclusion: The frequency of PAS is similar to other populations. Prior cesarean delivery, placenta previa, multigravidity, and multiparity were independent risk factors and also perinatal hysterectomy and preterm birth were the most important complications.

Background: Episiotomy is one of the common procedures during childbirth in Iran, which causes pain, discomfort, and scar in the perineum. This study aimed to pursue the effect of Malva Sylvestris cream on episiotomy pain and healing. Methods: This study was a double-blinded randomized-controlled clinical trial that was conducted from April to December 2021 at the Sina hospital in Ahvaz, Iran. Sixty women were selected and randomly assigned to control and intervention groups using block randomization. The main outcomes included pain assessment and episiotomy wound healing that are assessed by the Visual analog scale (VAS), and perineal healing scale included redness, edema, ecchymosis/bruising, discharge, approximation (REEDA). The cream was used twice a day for up to 14 days. The participants were followed on the first, seventh, and fourteenth days postpartum. Independent T-test, Mann-Whitney, and Chi-square, and Generalized Estimating Equations (GEE) model were used by SPSS software version 22 for data analysis. Statistically significant level was considered less than 0.05. Results: There were no significant statistical differences between the two groups in demographic characteristics (P>0.05). No significant statistical differences were found in both groups in terms of perineal healing (B=-0.05; P=0.89) and pain scores (B=0.15; P=0.56). However, the chance of external dysuria in the intervention group decreased by 77% (P=0.01). Conclusion: Despite showing the positive effect of Malva Sylvestris extract on wound healing in animal and in-vitro studies, this clinical study failed to show the positive effect of this extract on wound healing and pain relief of episiotomy. However, future clinical trials are needed to substantiate the above findings.Trial Registration Number: IRCT20190826044621N1.

BACKGROUND: Asthma and allergic complications are the most common chronic disorders in children and adolescents. This study aimed to determine the prevalence and severity of asthma, allergic rhinitis, eczema among schoolchildren, and some related risk factors. METHODS: The cross-sectional study was performed in 2019 and involved 4000 students aged 6-7 years and 4000 students aged 13-14 years (both girls and boys) from urban schools in Khuzestan Province, southwestern Iran. We used the multi-stage sampling method. Data were collected using the Persian version of the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire. RESULTS: The prevalence of current wheeze, wheeze ever and asthma ever in the age group of 6-7 years was 3.8, 4.5, and 1.8%, respectively; in the age group of 13-14 years, it was 4.4, 5.9, and 3.4, respectively. In terms of gender, males (4.9, 6.0, and 2.7 percent, respectively) had substantially higher rates of current wheeze, wheeze ever, and asthma ever compared to the girls (2.8, 3.6, and 2.0 percent, respectively) (p < 0.001). The prevalence of rhinitis, Hay fever, and eczema among young people was 5.3%, 3.5%, and 1.0%, respectively. Current wheeze was more common in children with allergic rhinitis in the last 12 months (30.3% vs. 2.7%, p < 0.001), Hay fever (32.8% vs. 0.3%, p < 0.001) and eczema (27.8% vs 3.8%, p < 0.001), compared to children who did not. CONCLUSIONS: The prevalence and severity of asthma symptoms were significantly associated with allergic rhinitis, eczema, and gender.

After the early rainfall in the autumn of 2013, respiratory syndromes spread in the Khuzestan province of Iran with the most severity in Ahvaz. There have been recurring outbreaks in recent years. Considering that pollen-derived airborne allergens are regarded as key aeroallergens and the main cause of allergic rhinitis and asthma, this work aimed to forecast total pollen concentration in Ahvaz through an artificial neural network (ANN), followed by evaluating the pollen spatial distribution across the city and the association between pollen concentrations and environmental parameters. The utilized ANN in this work included an input layer with 13 parameters, a hidden layer of five neurons, and an output layer. Data were classified into training, validation, and testing sets. The ANN was implemented with 70% and 80% of data for training. The value of the correlation coefficient for the data validation of these two networks was 0.89 and 0.92, respectively. The results also indicated that despite the difference in the mean concentration of the pollens in various areas of Ahvaz, this difference was not statistically significant (P > 0.05). Furthermore, there was a negative correlation between the concentration of total pollen and relative humidity, precipitation, and air pressure. However, it had a positive correlation with temperature. Consequently, considering the logistical challenges of monitoring bioaerosols in the air, the ANN approach could predict total pollen concentrations. Therefore, in addition to measurements, the ANN technique can be a good tool to enable authorities to mitigate the impact of airborne pollen on people.

The presence of criteria air pollutants (CAP) in the ambient air of a populated inhalation region is one of the main serious public health concerns. The present study evaluated the number of cardiovascular mortalities (CM), hospital admissions with cardiovascular disease (HACD), and hospital admissions for respiratory disease (HARD) due to CAP exposure between 2010 and 2014. The study used the Air Q model and descriptive analysis to investigate the health endpoint attributed to the ground level of ozone (O3), nitrogen dioxide (NO2), sulfide dioxide (SO2), and particle matter (PM10). Baseline incidence (BI) and relative risk (RR) are the most important factors in the evaluation of health outcomes from exposure to CAP in the ambient air of a populated area according to EPA and the World Health Organization (WHO) guidelines. Our study showed that annual cases of cardiovascular mortality during the period 2010-2014 relating to particle mater were 478, 506, 469, 427, and 371; ozone was 19, 24, 43, 56, and 49; nitrogen dioxide was 18, 20, 23, 27, and 21; and sulfide dioxide was 26, 31, 37, 43 and 11, in the years 2010 to 2014, respectively. These results indicate that the number of hospital admissions for respiratory disease attributed to PM were 2054, 2277, 2675, 2042, and 1895; O3 was 27, 35, 58, 73, and 63; NO2 was 23, 24, 15, 25, and 18; and SO2 was 23, 24, 25, 30, and 20, in the years from 2010 to 2014, respectively. The results also showed that the number of hospital admissions for cardiovascular disease related to particle mater was 560, 586, 529, 503, and 472; ozone was 22, 32, 38, 55, and 51; nitrogen dioxide was 19, 18, 13, 21, and 14; and sulfide dioxide was 12, 14, 16, 22, and 9, in the same period, respectively. Observations showed that most of the pollution was from outdoor air and in the human respiratory tract. Increased levels of sulfide dioxide, particle matter, nitrogen dioxide, and ozone can cause additional morbidity and mortality for exposed populations. According to the results, it is possible to help increase the level of public health. The use of these findings could also be of great help to health professionals and facilitators at regional and national levels.

Differentiation between exudative and transudative pleural effusion is sometime problematic. This study aimed to evaluate the diagnostic value of C-reactive protein (CRP) in differentiation of exudative and transudative pleural effusion. This is an analytical epidemiologic cross-sectional study that evaluates the role of CRP in differentiating transudative and exudative pleural effusion. Patients were divided into two groups of exudates and transudates, based on Light's criteria. The pleural effusion CRP levels were compared between the two groups. SPSS software version 16 was used for statistical analysis. The significance level was considered p<0.05. A total of 169 patients with pleural effusion enrolled in the study. Based on Light's criteria, 108 patients (63.9%) had exudative pleural effusion and 61 (36.1%) had transudative pleural effusion. The level of CRP in the pleural fluid of patients in the exudative and transudative groups was 13.3±37.1 and 3.5±4.3mg/dl, respectively (p=0.008). The 3.31 mg/dl cut-off point of CRP level of pleural effusion had the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of 96.3%, 72.1%, 86% and 91.7% respectively.  The results obtained in our study shows that the level of CRP in the effusion fluid can be helpful in differentiating exudative from transudative pleural effusions.
